Anyone recommend a decent one? I'd prefer an all in one unit rather than
the type which uses a wall wart - as the power distribution to the AV
system is IEC, rather than 13 amp. So a unit where I can just fit an IEC
plug to would be the neatest way. Adjustable output level would be nice
too.

I do have one which fulfils those requirements, but does AES too, and
boots up in the wrong mode. ;-)

Well, apart from it using an external PS and being fixed output level. ;-)

My TV has a fixed output level to toslink. Not even the mute works on it.
Now decent TV sound only peaks to -10dBFS, so this convertor will sound
low level compared to other inputs. As it's not made to the EBU standard
for TV sound.

Hence my specific requirements.

Could you take the power from a TV USB port? That'd get you part way there . .

The sound system - mainly analogue with not a USB port in sight - is on
the other side of the room to the TV. And an optical cable already
installed between them. And I doubt USB volts would make it that far
anyway. ;-)

It looks like no one wants to spend too much of your money! The internal
power requirement really narrows the field.

The S.M.S.L M500 has internal power, an optical input and a preamp mode.

'installers'. Works fine.

Failing that, see if you can get a discounted example of the lowest CA DAC
Magic range before they cease to be available now the their 200M is
