I recently checked my install of Agony Unrated on Steam and found it
wanted me to "purchase" it. I thought, "WTF? I bought the unrated
version."

Turns out they moved "unrated" to a DLC, and I had to "buy" it. It was
free. Possibly because I had already purchased it, possibly because it's
now free to anyone.

I think it was a move to protect the kiddies from accidental anatomy
lessons.

Anyway, you may want to grab that particular DLC before they make it
paid. You may not.

Googling the issue, ths convoluted solution seems to be a reaction to
various national censorship laws.

There are two SKUs on Steam; the original "Agony" and a newer "Agony
UNRATED". The latter has received significant code and visual updates
over the original and so the developers wanted to sell it as an
updated version.

However, the naughty 'unrated' bits would have gotten the game banned
in some countries (Germany is frequently mentioned, which is
unsurprising as the BPjS / BPjM index is renowned for its strictness).
So to get around this - and ensure players could get the most updated
version - the 'unrated' parts were separated into a DLC. That way the
DLC could be banned while the main game slipped through.

Why they don't just backport the updates to the original SKU remains
unanswered. Hypothesis range from that it would be such a significant
rewrite of the original code that it's not worth the effort;
alternately, the developers are greedy pricks and want people to buy
their game twice. I've no information, and no opinion on the matter.

Looking at the game itself, the conceit is interesting but it feels
very try-hard in how extreme it is; sure, Hell is supposed to be an
awful place full of awful people, but the developers seem to take a
shamelessly sophomoric glee in unnecessarily showing all the worst
aspects of the place. I get a lot of "Postal" vibes from the game;
'Hurkhurkhurk, you can EAT a baby in our game; we're so EXTREME!'
Unfortunately, the gameplay, from what I've seen, doesn't look
particularly engaging either. So once you get past the shock value of
the piece I'm not sure there's any real point to playing it.
