We're seeing some inverted and reversed ultrasound images in our viewer. I'm trying to track down what the cause or solution might be. Any ideas welcome?

It is probably a weird combination of photometric interpretation and/or compression. There is an extensive test set of images available as part of the IHE CPI (Consistent Presentation of Images) profile that you can run through your viewer so you can identify any potential support issues, here is the link: https://gazelle.ihe.net/content/dicomscopeimagedisplay you will be looking for the imgdisplay_testcases.zip to download the test images and corresponding test document.

I don't suppose the US images originated from a big endian byte order system? I've seen cases where RGB images (US) get their color channels swapped incorrectly when decompressed because they moved between big and little endian systems.

Have you tried the images in other viewers? It's always good to cross check. What Transfer Syntax are the bad images? If it's JPEG Baseline, then you can extract one image frame, save as JPEG, and display in literally any photo viewer.
