Rob, is anyone tracking infection (hospitalization, death, etc.) rates as
differentiated by vaccination status (and, ideally, vaccine used)?

The only vaccine-type-related statistic that I see regularly reported
is VITT occurence rates for the AZ and J&J types..

The UK has collected info on post-vaccine-related results, sufficient
to convince most authorities that a single dose of any will reduce
Intensive Care Unit bed occupancy and fatality rates.

I believe that in the UK, vaccines are used 'as directed', with no
fuzzy definitions about second dose intervals, so this information
is drawn from ~30 day period dosing. Regions where this is assumed
to cover longer intervals between doses have yet to report useful
statistics - it would take more than 3x longer to do so.

So, they're only "worried" about "adverse effects".

You'd think htey would also try to harvest efficacy data -- have PCPs report
covid-like illness and whether or not the pt was vaccinated; ditto for
hospitals. In addition to fleshing out the *actual* (?) efficacy, it
would also provide an *insight* into how long immunity lasts.

Yet another "cost avoidance" issue. But, still doesn't attest to how
effective the vaccine(s) are, in practice.

Here (US) we should start to see data on the long-term effects
as we're already 3 months out for some pts. Waiting until ~9 months
to start noticing patterns seems a bit late...

The University of Birmingham released a study a week ago that looked
at 30day to 12 week immunity response of the Pfizer vaccine.

https://www.birmingham.ac.uk/news/latest/2021/05/covid-pfizer-vaccination-interval-antibody-response.aspx

So there is obviously some capacity to compare the effects of
different vaccines, where funding permits.

Admittedly, 'antibody response' is not likely to be one of the
typical features of a case study parameter; vaccination dates
hospitalization and fatality, however, couldn't fail to be
included in the UK's National Health records for the NIHR.

What I'm talking about is the relative ease of *collecting* the
data -- *if* someone mandates it. The analysis "funding"
is a separate issue.

Here (US), we have "reportable diseases" -- Dxs that require the
physician to "report" the Dx to the "local authorities". It is
typically used to track highly contagious diseases (i.e., you
want a "heads up" if you start to see a measles outbreak; you
don't want to rely on an individual doctor to know what may be
happening in other offices/clinics around town!). Or, diseases
that have other potential "public health" importance.

E.g., a case of Hanta Virus likely wants to be investigated
as soon as it is "reported" as the consequences of it getting out
of control are serious.

Reportable diseases have different reporting requirements
as befitting the risk posed by the disease incident.

In some cases, you (the medical practitioner, NOT the pt) are
just interested in a count. In other cases, you might have
to prepare a written accounting of the individual case.

I *think* most folks would have no problem considering covid to be
of significant "public health" importance! :>

So, it seems logical that making it reportable and requiring
"vaccination status" to be a part of that report to be a
simple way of *collecting* the data. Going *back*, in time,
to try to reconstruct the data would be an exercise in futility.
And, a significant burden on providers (if the pt presented at
an ER, would the ER have to track the pt down, later, to inquire
as to his immunization status AT THE TIME HE PRESENTED?)

If (as seems likely) covid becomes endemic, then any
data gathering opportunities that are overlooked seems
a foolish waste.

I don't know how it works in the UK under NHS, but
in Canada your contact with medical professionals or a
health care provider is documented in a patient-specific
record associated with a registration number - if the
Doctor, health care provider or pharmacist expects to
be reimbursed under the health care program for the
service(s) being provided.

Reportable illnesses require added paperwork. I'm not
sure who ends up paying for that administration time,
or who is looking at the results on any particular
schedule. Supposedly some (ie infectious diseases) are
considered more time-sensitive than others.

Here, I believe your "MRN" is only tied to your PCP.
I.e., if I go to a hospital in another town, they
just know me by my name; my "records" are likely not
accessible to that provider. (we have "islands" of
data, not a continuum)

The pt is ultimately responsible. So, the provider wants
a way of getting back to *you* (e.g., if your insurance
denies the claim, the provider isn't going to just eat it)

As everything in the US is "fee for service", I'm sure there
is a code for "report illness to local health agencies"
that the provider "checks".

Note that reporting may be as simple as placing a phone
call or as involved as documenting the individual case.

Regardless, when the gummit is absorbing all of these
other costs associated with covid, you'd think tacking on
that additional requirement (and fee) would be a no-brainer.

Note that I was discussing getting *treated* (Dx-ed) for
covid, not *vaccinated*. Though I believe the gummit
is (was?) underwriting much of the cost of *care*
during the pandemic (were providers required to report
pt identities?)

You already *do* "surrender privacy" to get vaccinated
(if you consider the fact that you have to provide
an ID -- the information on which is transcribed into
an "official record" -- to get the shot)

And, of course, the microchip that is contained in the
injected serum (to track your location, 24/7/365!)

Israel, with >120% of vaccine doses applied to vaccinate 59% of
the population, reports a daily virus detection rate below 10ppm
and a 0.4ppm/day fatality rate. These figures have not altered
significantly in the last 30 days - indicating the end of
Pfizer's vaccination trial.

The US, UK and Chile have all issued >80% of vaccine doses, with
vaccination rates of 39%, 33% and 40% respectively. Detection
and fatality rates, however, do not correlate:

those of Israel, which benefits from 2x their vaccination levels.
- The US has only achieved detection and fatalty rates comparable
to the EEC, which benefits from < 1/3 of their vaccination levels.
- Chile seems to have benefited only in that it's fatality rates
are reduced from those of its unvaccinated neighbour, Peru.
Detection rates in Chile are actually rising.

The US, UK and Chile have all issued >85% of vaccine doses, with
vaccination rates of 39%, 35% and 41% respectively. Detection

to the EEC, which benefits from < 1/3 of their vaccination levels.
- Chile seems to have benefited only in that it's fatality rates
are reduced from those of its unvaccinated neighbours, Peru and
Argentina. Detection rates in Chile have risen consistently for
the last 3 weeks.

So glad to see "it's just like another flu!" (not)

Am I misreading something? It seems like the fatality rate in UK is
considerably HIGHER than in Israel *and* the US.
UK 2.9%
US 1.8%
IL 0.8%
Are you (they?) using "fatality rate" in two different ways (fatalities
per unit of population vs. fatalities per detected infection)?

Also, it seems like the nominal ~2% fatality rate is reasonably consistent
(throughout the pandemic) -- except for some FAR outliers. E.g., MX at 9.5%.
Is this possibly due to underreporting of infections due to lack of testing?
(24ppm/day)

I'm refering to daily fatalities, sorry for any confusion.

US 1.6ppm/day
UK 0.1ppm/day
Ch 5.7ppm/day
Il 0.2ppm/day

Daily fatalities tend to follow detection trends by a week or
two.

Fatality rates are, of course, longer term calculations that
don't reflect ongoing trends. They should probably be limited
to 12 month or other specific time periods.

The US, UK and Chile have all issued >85% of vaccine doses, with
vaccination rates of 41%, 41% and 44% respectively. Detection
and fatality rates, however, do not correlate:
- The UK has achieved detection and fatality rates comparable to
those of Israel, which benefits from 2x their vaccination levels.
- The US has only achieved detection and fatalty rates comparable
to the EEC, which benefits from < 1/3 of their vaccination levels.
- Chile seems to have benefited only in that it's fatality rates
are reduced from those of its unvaccinated neighbours, Peru and
Argentina. Detection rates in Chile have risen consistently for
the last 3 weeks.

- . . . and what the hell is going on in Argentina, anyways?

- Peru has recently doubled it's covid fatality count. Can only
hope that it's some kind of typo.

The US, UK and Chile have all issued >85% of vaccine doses, with
vaccination rates of 42%, 42% and 45% respectively. Detection
and fatality rates, however, do not correlate:

It's not
a typo, but a correction in record keeping.
